Please note that internet access will not be provided for your session (except for technology sessions), but you will be able to order it if needed. Technology Sessions will be provided with an LCD projector and required equipment, if requested above.

 

Agreement

* I understand that as a clinician, I will be expected to register and pay the registration fee for the 2014 MMC, and will be responsible for all of my own expenses. If I fail to register before the "early deadline," I will pay the late registration fee.


* I understand that if I need equipment not supplied by the MMC, it can be ordered for me, and I will be invoiced for this equipment prior to the conference. Technology sessions will be provided with an LCD projector and required equipment, if requested above.


* If my proposal is accepted, any information provided by me may be used by the MMC for publicity and publication purposes.
